Having a good care plan will improve one’s overall quality of life, enabling the elderly to live independently for as long as possible. While advance care planning takes care of the future care needs of an elderly, it is incomplete if LPA and AMD are not taken care of too. To do this well, one has to plan ahead for their future healthcare needs while they are still able to make decisions and communicate decisions, and not be taken for granted when they no longer have the mental capacity to make decisions for themselves due to health conditions or other circumstances.
